There is no time restriction and no limit to what you can earn. Simply complete and print this form and return to us by mail with your original receipt(s) and one complete product UPC (bar code) clipped from each different product package. You will receive your rewards item(s) within 3 weeks. We will do our best to keep all listed rewards items on hand; however, if we should happen to run short on your particular item, we will notify you of any delay.
